Job summary

Blue Circle Rehab is seeking a dedicated Physical Therapist (PT) PRN to provide expert physical therapy services to geriatric inpatient clients. You will develop personalized treatment plans, perform evaluations, and deliver interventions across outpatient, inpatient, and home health settings to improve mobility and quality of life.

Responsibilities include comprehensive patient evaluations, discharge planning, and collaborative care with physicians, nurses, and therapists.

Qualifications

• Proven clinical experience as a Physical Therapist, with geriatrics emphasis
• Experience delivering discharge planning and outpatient/inpatient rehab
• Strong documentation and EHR/EMR familiarity
• Hands-on therapy skills including manual therapy and therapeutic exercises

• Collaborate with physicians, nurses, occupational therapists, and other team members to coordinate care and discharge planning.
• Monitor patient progress and adjust treatment plans to maximize recovery and rehabilitation outcomes.
• Maintain accurate EHR/EMR records for all patient interactions and documentation.
• Educate patients and families on treatment procedures, home exercises, safety measures, and community resources.
• Participate in team meetings to review treatment strategies and contribute to multidisciplinary care planning.

Duties

Assess patients’ physical conditions through comprehensive evaluations, including physiology and anatomy knowledge.

Develop and implement individualized care plans aligned with clinical documentation standards and best practices.

Deliver a variety of therapeutic interventions such as therapeutic exercises, trigger point therapy, manual therapy, and modalities tailored to patient needs.

Collaborate closely with healthcare professionals—including physicians, nurses, occupational therapists—to coordinate care and discharge planning.

Monitor patient progress regularly; adjust treatment plans accordingly to maximize recovery and rehabilitation outcomes.

Maintain accurate electronic health records (EHR) and electronic medical records (EMR) systems for all patient interactions and documentation.

Educate patients and their families on treatment procedures, home exercises, safety measures, and community resources to support ongoing recovery.

Participate in team meetings to review treatment strategies, share insights on patient progress, and contribute to multidisciplinary care planning.

Experience

Proven experience practicing in a physical therapy role within outpatient clinics or inpatient hospital settings.

Demonstrated expertise in discharge planning, treatment plan development, and patient rehabilitation strategies.

Hands-on experience working with geriatric patients

Familiarity with clinical documentation standards and medical terminology necessary for accurate record keeping.

Experience managing electronic health records (EHR) systems or electronic medical records (EMR) platforms efficiently.

Strong knowledge of physiology and anatomy relevant to physiotherapy practices across diverse patient populations.

Ability to collaborate effectively with healthcare professionals across disciplines while maintaining excellent client communication skills.

Prior outpatient rehabilitation center or outpatient clinic experience is highly desirable; inpatient hospital experience is a plus.

Knowledge of physiotherapy techniques such as trigger point therapy and modalities used in outpatient or inpatient settings is beneficial.
