Job summary

Blue Central Group in New York is seeking a full-time Physical Therapist to deliver expert physiotherapy across inpatient hospital units, outpatient clinics, home health, and community programs. You will assess, develop and implement care plans, educate patients and families, and collaborate with multidisciplinary teams to improve mobility and reduce pain.

This role requires valid PT licensure, acute care experience, and a compassionate approach to patient-centered care, including EMR

Qualifications

• Licensed Physical Therapist with active licensure in the relevant jurisdiction.

Duties

• Assess patients’ physical conditions by conducting comprehensive evaluations of mobility, strength, and functional abilities
• Develop and implement tailored care plans that align with patients’ goals and medical needs
• Perform therapeutic interventions such as trigger point therapy, orthopedics techniques, and geriatrics-focused exercises to promote recovery
• Manage discharge planning processes to ensure seamless transition from hospital or clinic to home or community settings
• Document patient progress accurately using electronic medical record (EMR) systems and communicate updates with healthcare teams
• Provide specialized care for pediatric, geriatric, inpatient, outpatient, and home health populations
• Educate patients and caregivers on physiotherapy exercises, injury prevention strategies, and health maintenance practices

Requirements

• Valid licensure as a licensed Physical Therapist (PT) in the relevant jurisdiction
• Proven experience in acute care, hospital environments, outpatient clinics, or home & community care settings
• Strong knowledge of physiology, anatomy, medical terminology, and orthopedic principles
• Demonstrated ability to develop effective care plans for diverse patient populations including pediatrics and geriatrics
• Familiarity with EMR systems for documentation and patient management
• Excellent patient care skills with a compassionate approach to treatment and education
• Ability to work independently and collaboratively within multidisciplinary teams
