Physical Therapist (PT) PRN - Duties and Responsibilities:

1. Responsible for the assessment of referred patients requiring rehabilitation services.
2. Providing direct patient care to assess their medical condition, functional capabilities, limitations, restrictions, and rehabilitation potential.
3. Establish and administer a treatment program with specific goals based on the patient's capacity and tolerance under the physician's direction.
4. Set realistic and achievable goals for patients.
5. Document and record patients’ conditions and educate patients and families in appropriate physical therapy methods.
6. Complete evaluations, weekly progress notes, recertification, and discharge documentation.
7. Collaborate with all disciplines to provide patient-centered treatment and care.
8. Perform all other duties assigned by the Director of Rehabilitation.
9. Ensure compliance with confidentiality, HIPAA, and Federal Residents' Rights.

Requirements/Qualifications:

• Graduated from an accredited Physical Therapy college or university.
• Valid professional license and registration for the state of practice.
• Experience with long-term care and/or geriatric care preferred but not required.
• Experience with Case Mix Index and PPS scheduling preferred but not required.
