Job summary

Kindred Hospital Rancho Cucamonga is seeking a Physical Therapist (PT) for a per diem position at a Long-Term Acute Care Hospital in Rancho Cucamonga, CA. The role focuses on evaluating patients, developing treatment plans, and guiding progress day by day.

You will instruct nursing staff and families on follow-through programs, document care per regulatory requirements, and maintain a safe, organized work environment. Strong communication and organizational skills are essential.

Qualifications

• Degree from an accredited Physical Therapy program.
• Current and unrestricted Physical Therapy license in the state of service.
• Current CPR certification.
• Strong organizational and communication skills.

Responsibilities

• Evaluate a patient's condition, develop a treatment plan, and guide day-by-day progress.
• Instruct nursing staff and patient families on follow-through programs.
• Communicate patient progress to supervisor and team; assist with scheduling and post charges.
• Document patient care according to regulatory, licensing, payer and accrediting requirements.
• Instruct patient’s family or nursing staff in follow-through programs.
• Maintain equipment and work area in a safe, clean condition.
• Adhere to Company Code of Conduct and applicable laws.

How you'll contribute

• Put your physical therapy skills to work where they're really needed -evaluate a patient's condition, develop a treatment plan, and help them get better, day by day. You'll also instruct the nursing staff and the patient's families on follow-through programs that build on the progress they've made
• Communicate patient progress or problems to supervisor and other team members; assist with patient scheduling and post charges daily to patient records
• Document patient care in accordance with regulatory, licensing, payer and accrediting requirements
• Instruct patient's family or nursing staff in follow-through programs
• Maintain equipment and work area in a safe and clean condition
• Handle job responsibilities in accordance with the Company's Code of Business Conduct, the Corporate Compliance Agreement, appropriate professional standard and applicable state/federal laws
• Other duties as assigned
