Job summary

Orange Park Rehabilitation and Nursing Center is seeking a licensed Physical Therapist to lead screening, evaluation, and treatment of facility patients needing PT services.

The role includes reviewing medical information, developing and implementing treatment plans, educating staff, supervising PT assistants, and participating in interdisciplinary meetings and family conferences. Qualifications

  • Must hold a current state license in Florida.
  • Degree in Physical Therapy from an accredited program.
  • Excellent oral and written communication skills.
  • Demonstrated competency in therapy treatment and evaluation methods.

Responsibilities

  • Assumes responsibility for screening, evaluation, and treatment of facility patients/residents needing Physical Therapy services.
  • Reviews medical information and conducts screenings prior to evaluations to determine patient need and benefit.
  • Performs evaluations under physician order and develops a treatment plan.
  • Implements treatment plan through direct treatment and staff education, supervising PT staff.
  • Participates in continuing education and other learning opportunities.
  • Maintains confidentiality of patient, facility, and physician information; handles daily billing per procedures.
  • Attends interdisciplinary meetings to relay clinical findings and recommendations.
  • Participates in family meetings as appropriate; adheres to department policies.
  • Conducts staff training via in-services and consultations; follows Rehab policy and procedure manual.
  • Supervises PT Assistants and rehab aides/extenders; supervises non-professional staff as indicated by state laws.

Job description

Qualifications: The Physical Therapist must: have a Bachelors and/or Master’s Degree in Physical Therapy, have a current state license, have excellent oral and written communication skills, demonstrate competency in therapy treatment and evaluation principles, techniques, materials, and equipment utilization, and demonstrate knowledge of state and federal accreditation standards.

The Physical Therapist:
  • Assumes responsibility for the screening, evaluation, and treatment of facility patients/residents needing Physical Therapy services
  • Reviews medical information and conducts screenings prior to evaluations to determine patient need for services and ability to benefit from skilled services
  • Performs patient’s evaluations under physician order and develops a treatment plan in accordance with patient’s needs
  • Implements treatment plan through direct treatment, education of treatment staff, and supervision of physical therapy staff
  • Assumes responsibility for continued professional growth and development by participating in continuing education programs and other appropriate learning experiences
  • Maintains confidentiality of all patient, facility, and physician-related information
  • Maintains accurate billing information, in accordance with procedures, on a daily basis
  • Attends and participates in interdisciplinary meetings, as appropriate, to relay clinical findings and recommendations to other members of the rehab team
  • Attends and participates in family meetings, as appropriate
  • Adheres to documentation requirements of the Rehabilitation department and the facility
  • Conducts staff training/education, as appropriate, via formal and informal in-services and consultations
  • Reviews and follows the policies and procedures outlined by the facility and Rehab Policy and Procedure Manual
  • Supervises Physical Therapy Assistants and rehabilitation aides/extenders as indicated by patient needs and state laws and discipline practice act
  • Supervises non-professional staff, as appropriate
