Job summary

Topaz Fiscal Services is seeking a licensed Physical Therapist to screen, evaluate, and treat facility patients/residents in need of skilled Physical Therapy services. You will review medical information, develop and implement treatment plans, and supervise therapy staff to ensure quality care.

Responsibilities include maintaining confidentiality, documenting daily billing, and participating in interdisciplinary and family meetings.

Qualifications

  • Must have Bachelor's, Master's, or Doctorate Degree in Physical Therapy.
  • Must hold a current state license.
  • Excellent oral and written communication skills.
  • Demonstrates competency in therapy treatment and evaluation principles, techniques, materials, and equipment utilization.
  • Demonstrates knowledge of state and federal accreditation standards.

Responsibilities

  • Screen, evaluate, and treat facility patients/residents needing Physical Therapy services.
  • Review medical information and conduct screenings to determine patient needs.
  • Perform patient evaluations under physician order and develop treatment plans.
  • Implement treatment plans and supervise therapy staff as needed.
  • Participate in continuing education to maintain professional growth.
  • Maintain confidentiality of patient, facility, and physician information.
  • Maintain accurate billing information daily.
  • Attend interdisciplinary meetings to relay clinical findings and recommendations.
  • Attend family meetings as appropriate.
  • Adhere to documentation requirements of Rehabilitation department and facility.
  • Conduct staff training via in-services and consultations.
  • Review policies and procedures of the facility and Rehab Manual.
  • Supervise Physical Therapy Assistants and rehab aides/extenders per state laws.
  • Supervise non-professional staff as appropriate.

Job description

Qualifications: The Physical Therapist must: have a Bachelor's, Master’s, or Doctorate Degree in Physical Therapy, have a current state license, have excellent oral and written communication skills, demonstrate competency in therapy treatment and evaluation principles, techniques, materials, and equipment utilization, and demonstrate knowledge of state and federal accreditation standards.

The Physical Therapist:
  • Assumes responsibility for the screening, evaluation, and treatment of facility patients/residents needing Physical Therapy services
  • Reviews medical information and conducts screenings prior to evaluations to determine patient need for services and ability to benefit from skilled services
  • Performs patient’s evaluations under physician order and develops a treatment plan in accordance with patient’s needs
  • Implements treatment plan through direct treatment, education of treatment staff, and supervision of physical therapy staff
  • Assumes responsibility for continued professional growth and development by participating in continuing education programs and other appropriate learning experiences
  • Maintains confidentiality of all patient, facility, and physician-related information
  • Maintains accurate billing information, in accordance with procedures, on a daily basis
  • Attends and participates in interdisciplinary meetings, as appropriate, to relay clinical findings and recommendations to other members of the rehab team
  • Attends and participates in family meetings, as appropriate
  • Adheres to documentation requirements of the Rehabilitation department and the facility
  • Conducts staff training/education, as appropriate, via formal and informal in-services and consultations
  • Reviews and follows the policies and procedures outlined by the facility and Rehab Policy and Procedure Manual
  • Supervises Physical Therapy Assistants and rehabilitation aides/extenders as indicated by patient needs and state laws and discipline practice act
  • Supervises non-professional staff, as appropriate
