Job summary

Reedy River Post Acute in Greenville, SC is seeking a Physical Therapist (PT) to evaluate and treat residents to improve mobility, function, and independence. You will collaborate with an interdisciplinary team to deliver high-quality, patient-centered care in a skilled nursing setting.

The role offers PRN opportunities with competitive pay ($40-$45/hour) and a supportive, team-oriented rehab environment. South Carolina license required; prior SNF or rehab experience preferred.

Qualifications

• Degree in Physical Therapy required; Bachelor's is mandatory, advanced degrees preferred.
• Active, unencumbered Physical Therapist license in South Carolina.
• Experience in a skilled nursing or rehab setting preferred.
• Familiarity with PCC and Casamba is advantageous.
• Strong clinical, leadership, and communication abilities.

Responsibilities

• Perform patient evaluations and develop individualized treatment plans.
• Provide therapeutic interventions to improve strength, balance, mobility, and function.
• Monitor patient progress and adjust treatment plans as needed.
• Maintain accurate and timely clinical documentation.
• Communicate with physicians, families, and care team members.
• Participate in care planning, discharge planning, and rehab meetings.
• Supervise PTAs, therapy aides, and students.
• Provide in-service training and education for staff.
