Job summary

Woodland Manor Nursing and Rehab in Elkhart, IN is seeking a compassionate Physical Therapist (PT) to join our rehabilitation team on a PRN basis. You will evaluate residents, develop treatment plans, and implement evidence-based therapies to improve mobility, strength, and independence while supporting a patient-centered care approach.

You will collaborate with an interdisciplinary team, document progress, and educate residents and families on exercises.

Qualifications

• Licensed PT in Indiana.
• Current BLS certification.
• Strong clinical judgment and communication skills.
• Compassionate, patient-focused approach to care.
• Ability to work independently while collaborating with rehab and interdisciplinary teams.

Responsibilities

• Evaluate residents and develop individualized physical therapy treatment plans.
• Provide evidence-based, patient-centered therapy to improve mobility, strength, balance, and independence.
• Monitor progress, update goals, and modify plans as needed.

What You'll Do:

• Evaluate residents and develop individualized physical therapy treatment plans

• Monitor resident progress, update goals, and modify treatment plans as appropriate
• Collaborate with the interdisciplinary care team to support coordinated and effective rehabilitation outcomes

• Educate residents, families, and caregivers on exercises and strategies that support continued progress and independence
