We are seeking to hire a Physical Therapist (PT) - Per Diem Float to provide trusted, compassionate, and expert home care to the community.

As a Homecare Physical Therapist - Per Diem Float, you will work with clients one on one in their home environment focusing on functional mobility, fall prevention, and home safety. You will develop and implement creative and evidenced based individual care plans that will enhance clients’ quality of life in their own home. Homecare is the leading edge of Geriatric Physical Therapy and must be comfortable implementing evidence-based interventions to this unique demographic.

Responsibilities for a Physical Therapist (PT) - Per Diem Float:

Assess client’s rehabilitation needs and potential to develop a physical therapy program which may include therapeutic exercises, gait training, use of adaptive equipment and management on orthotic and prosthetic devices
Collaborate with appropriate members of the health care team to provide physical therapy as well as other services determined necessary
Regularly participate in interdisciplinary case conference and document patient information as needed
Instruct clients and family members regarding rehabilitative therapy as part of the plan of care resulting in the client/family assuming increased responsibility for care when appropriate
Provide educational programs as required to prepare other corporation personnel to assist in client’s plan of treatment
Delegate appropriate tasks/assignments to PTA’s
Maintains a productivity average of 4.5 visits a day including completion of all necessary and accompanying documentation within 24 hours

Qualifications for a Physical Therapist (PT) - Per Diem Float:

NH PT License (DPT preferred)
Home Care experience beneficial, although we will train the right individual
Proof of Flu vaccination
Reliable transportation
Valid driver's license
Automobile insurance
Physical exam within the last year
Two-step TB test
