Job summary

Edgewood Center for Rehab is seeking a Per Diem Physical Therapist (PT) to join our rehab team at our 5-star skilled nursing facility in Portsmouth, NH. The role focuses on evaluating residents, developing individualized treatment plans, and delivering skilled therapy to restore function, improve mobility, reduce pain, and maximize independence.

You will collaborate with an interdisciplinary team, complete documentation, and support discharge planning while maintaining a safe, clean therapy

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current NH Physical Therapist license in good standing.
• Long-term care and/or geriatric experience preferred.

Responsibilities

• Evaluate residents and establish individualized treatment plans
• Provide skilled physical therapy interventions and therapeutic exercises
• Complete accurate and timely documentation
• Collaborate with interdisciplinary team members to support resident outcomes
• Monitor resident progress and modify treatment plans as appropriate
• Educate residents, families, and staff on mobility, safety, and exercise programs
• Assist with discharge planning and equipment recommendations
• Maintain a safe, clean, and organized therapy environment
• Support a positive and collaborative rehab culture

Education

Graduate of an accredited Physical Therapy program

Current NH Physical Therapist license in good standing

Long-term care and/or geriatric experience preferred
