Job summary

Baptist Health Shelby Hospital in Alabaster, AL is seeking an Occupational Therapist or Physical Therapist to join our outpatient rehab team on a day shift. This full-time position (36 hours per week) offers a supportive, patient-centered environment in a modern acute care setting.

You will evaluate patients, develop individualized treatment plans, document progress and ensure regulatory compliance while collaborating with physicians, nurses and therapists.

Qualifications

• Bachelor’s degree or higher from an approved program in occupational therapy or physical therapy.
• Maintains current Occupational Therapist or Physical Therapist license in the State of Alabama; temporary permit acceptable; maintains BLS/Healthcare Provider certification.
• 1+ year of OT or PT experience; Lymphedema certification required.

Responsibilities

• Evaluates patients, gathering data from the patient, physician, interdisciplinary team meetings if available, family and data sources.
• Develops treatment plans which integrate with the continuum of care and have specific goals and objectives.
• Demonstrates knowledge and skills necessary to treat and provide care appropriate to the age of the patients served on the assigned caseload.
• Maintains complete documentation and accurate billing; adheres to state practice acts and standards.
• Provides patient and family education and participates in program development and meetings.
