Qualifications

• Doctorate in Physical Therapy required; must maintain continuing education.

Schedule Details/Additional Information:

Located in beautiful Lake Zurich!

Offering a $2500 sign-on bonus payable in two installments: 50% after 90 days and 50% after one year.

Includes one evening shift until 7:00 p.m.

Responsibilities:

• Interpret physician referrals and perform evaluations (musculoskeletal, neuromuscular, cardiopulmonary, integumentary).
• Develop and implement individualized treatment plans, monitor progress, and adjust as needed.
• Coordinate discharge planning, patient education, and referrals.
• Complete all documentation accurately and timely, including evaluations, progress notes, and discharge summaries.
• Maintain a clean and safe treatment environment.
• Participate in performance improvement activities.
• Manage caseload and assist with coverage as needed.
• Pursue ongoing professional development.
• Supervise assistants and students as applicable.

Minimum Job Requirements:

Education:

Doctorate in Physical Therapy from an accredited institution or equivalent; maintain continuing education.

Certification / License:

Licensed Physical Therapist in the state; BLS certification.

Work Experience:

No experience required.

Knowledge / Skills / Abilities:

Intermediate computer skills, excellent communication, organizational, decision-making, and problem-solving skills. Knowledge of growth and development principles and ability to provide age-appropriate care.

Physical Requirements and Working Conditions:

Must be able to sit, stand, walk, lift up to 50 lbs., push/pull, and perform manual tasks as described. Must have functional senses for communication and safety. May be exposed to hazards and require travel.
