Job summary

Centra Healthcare Solutions, Inc. seeks an experienced or newly graduated Physical Therapist (PT) to join an outpatient facility in Livermore, CA.

This direct-hire, daytime position offers a stable full-time schedule. The PT will develop care plans, provide skilled PT services per physician orders, assess patient needs, modify treatments as needed, and collaborate with the multidisciplinary team to reach patient goals.

Qualifications

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist / PT or actively in the license application process.
• Current CPR certification.

Responsibilities

• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services / interventions in accordance with physician orders.
• Assesses patient needs, plans for, evaluates and modifies care to meet goals of physical therapy interventions.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.
