Job summary

Centra Healthcare is seeking an experienced or newly graduated Physical Therapist (PT) for an outpatient contract assignment in Phoenix, AZ. The role focuses on delivering skilled PT services and developing care plans in line with the clinic's mission and goals.

Ideal candidates have graduated from an accredited program, hold a current state PT license (or are in the licensure process) and possess CPR certification. This is a contract position with day-shift hours.

Qualifications

• Graduated from an accredited school.
• Current state PT license or actively pursuing licensure.
• Current CPR certification.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services per physician orders.
• Assess patient needs, plan, evaluate and modify care to meet goals.
• Collaborate with disciplines to plan and evaluate team goals.

A great Outpatient is looking for an experienced or newly graduated Physical Therapist / PT to join their growing team for a contract assignment! This is a contract position.Days schedule.

This role assumes responsibility and accountability for incorporating the vision, values, mission and critical goals of the organization into their job performance.

Requirements include:

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist / PT or proactively in the process of the application process for current state licensure as a Registered Physical Therapist / PT.
• Current CPR certification.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services / interventions in accordance with physician orders.
• Assesses patient needs, plans for, evaluates and modifies care to meet goals of physical therapy interventions.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.
