Job summary

Centra Healthcare is seeking a licensed Physical Therapist (PT) for a contract assignment in Norfolk, VA. The position requires current VA licensure or active licensure in process and a CPR certification.

You will join a SNF/LTC setting providing patient-centered PT services under physician orders. Responsibilities include developing care plans, delivering skilled interventions, assessing patient needs, and collaborating with the multidisciplinary team to meet individual goals.

Qualifications

• Must be licensed as a Physical Therapist in Virginia.
• Current CPR certification.
• Graduate from an accredited PT program.
• Develop a plan of care for each patient and provide PT services per orders.
• Collaborate with disciplines to meet patient goals.

Responsibilities

• Provide skilled PT services per physician orders.
• Assess patient needs and modify care plans as needed.
• Document progress and maintain patient records.

A great SNF / LTC is looking for an experienced or newly graduated Physical Therapist / PT to join their growing team for a contract assignment!

• This is a contract position.
• Days schedule.
• This role assumes responsibility and accountability for incorporating the vision, values, mission and critical goals of the organization into their job performance.

Requirements include

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist / PT or proactively in the process of the application process for current state licensure as a Registered Physical Therapist / PT.
• Current CPR certification.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services / interventions in accordance with physician orders.
• Assesses patient needs, plans for, evaluates and modifies care to meet goals of physical therapy interventions.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.
