Centra Healthcare Solutions is seeking a licensed Physical Therapist (PT) for a contract assignment in Newberry, SC, in a SNF/LTC setting. The role requires an active PT license and CPR certification, with both new grads and experienced therapists welcome.

You will develop care plans, provide skilled PT services per physician orders, assess patient needs, and collaborate with multidisciplinary teams to achieve therapy goals; this is a contract position with weekly pay.

Qualifications

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist / PT or in process of licensure.
• Current CPR certification.

Responsibilities

A great SNF / LTC is looking for an experienced or newly graduated Physical Therapist / PT to join their growing team for a contract assignment!

• This is a contract position.


• Days schedule.


• This role assumes responsibility and accountability for incorporating the vision, values, mission and critical goals of the organization into their job performance.

Requirements include:

• Must have graduated from an accredited school.


• Current state license as a Registered Physical Therapist / PT or proactively in the process of the application process for current state licensure as a Registered Physical Therapist / PT.


• Current CPR certification.


• Develop a plan of care for each physical therapy patient.


• Provide skilled physical therapy services / interventions in accordance with physician orders.


• Assesses patient needs, plans for, evaluates and modifies care to meet goals of physical therapy interventions.


• Collaborates with all disciplines to plan and evaluate team goals for each patient.
