Centra Healthcare Solutions is seeking a licensed Physical Therapist (PT) for a contract assignment in Atlanta, GA. The role focuses on providing skilled physical therapy services in a SNF/LTC setting, with responsibilities to develop and implement patient care plans and collaborate with a multidisciplinary team.

Requirements include graduation from an Accredited PT program, active state PT license (or in process), and CPR certification.

Qualifications

• Must have graduated from an accredited PT program.
• Current state license as a Registered Physical Therapist (PT) or in the process of licensure.
• Current CPR certification.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services/interventions per physician orders.
• Assesses patient needs, plans for, evaluates and modifies care to meet PT goals.
• Collaborates with all disciplines to plan and evaluate team goals.

Job Description:

A great SNF / LTC is looking for an experienced or newly graduated Physical Therapist / PT to join their growing team for a contract assignment!

• This is a contract position.
• Days schedule.
• This role assumes responsibility and accountability for incorporating the vision, values, mission and critical goals of the organization into their job performance.

Requirements include:

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist / PT or proactively in the process of the application process for current state licensure as a Registered Physical Therapist / PT.
• Current CPR certification.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services / interventions in accordance with physician orders.
• Assesses patient needs, plans for, evaluates and modifies care to meet goals of physical therapy interventions.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.
