Job summary

Centra Healthcare is seeking a licensed Physical Therapist for a contract assignment in Fayetteville, NC. The PT will evaluate patients, develop plans of care, and provide skilled physical therapy services per physician orders in SNF/LTC settings.

Candidates must graduate from an accredited program, hold an active PT license (or in licensure process), and collaborate with interdisciplinary teams to meet patient goals. This contract role provides weekly pay and benefits.

Qualifications

• Must be graduate of an accredited Physical Therapy program.
• Current state license as a Registered Physical Therapist (PT) or in process of licensure.
• Develop a plan of care for each patient.
• Provide skilled physical therapy services per physician orders.
• Assess patient needs, plan, evaluate and modify care for PT goals.
• Collaborate with all disciplines to plan and evaluate team goals.

A great SNF / LTC is looking for an experienced or newly graduated Physical Therapist / PT to join their growing team for a contract assignment!

• This is a contract position.
• Days schedule.
• This role assumes responsibility and accountability for incorporating the vision, values, mission and critical goals of the organization into their job performance.

Requirements include

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist / PT or proactively in the process of the application process for current state licensure as a Registered Physical Therapist / PT.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services / interventions in accordance with physician orders.
• Assesses patient needs, plans for, evaluates and modifies care to meet goals of physical therapy interventions.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.
