Job summary

Centra Healthcare is seeking a licensed Physical Therapist (PT) for a SNF/LTC setting in Elkhart, IN. This direct hire, permanent role requires an active PT license and CPR certification, with a daytime schedule.

You will develop a care plan for each patient, provide skilled PT services per physician orders, and collaborate with the care team to meet patient goals while aligning with the organization’s mission.

Qualifications

• Graduate from an accredited school.
• Current state license as a Registered Physical Therapist (PT) or in process.
• Current CPR certification.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services per physician orders.
• Assesses patient needs, plans for, evaluates and modifies care to meet goals of physical therapy interventions.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.

A great SNF / LTC is looking for an experienced or newly graduated Physical Therapist / PT to join their growing team!

Responsibilities

• This is a direct hire / 'perm' position.
• Days schedule.
• This role assumes responsibility and accountability for incorporating the vision, values, mission and critical goals of the organization into their job performance.

Requirements

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist / PT or proactively in the process of the application process for current state licensure as a Registered Physical Therapist / PT.
• Current CPR certification.
