Job summary

Centra Healthcare Solutions Inc. is seeking a qualified Physical Therapist (PT) for contract work to develop and implement plans of care for patients in a skilled nursing/long-term care setting.

The role requires graduation from an accredited PT program and an active state license (or active licensure application). You will collaborate with a multidisciplinary team to achieve patient-centered goals and deliver physician-ordered interventions.

Qualifications

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist / PT or actively pursuing licensure.

Responsibilities

• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services / interventions in accordance with physician orders.
• Assesses patient needs, plans for, evaluates and modifies care to meet goals of physical therapy interventions.
