Job summary

BAYADA Home Health Care is seeking a Physical Therapist to perform home health visits in the San Manuel, Oracle, Mammoth Territory in Arizona. You’ll deliver client-centered therapies in patients’ homes, promoting independence and safety with support from a seasoned BAYADA team.

The role emphasizes autonomy, ongoing clinical mentorship, and a collaborative culture focused on high‑quality care and professional growth. Flexible scheduling and strong onboarding are available to help you thrive.

Qualifications

• Must hold current Arizona PT license (or be eligible).
• Graduation from an accredited PT program.
• Comfort with EMR systems (training provided).

Responsibilities

• Perform mobility, strength, balance, and safety assessments.
• Develop individualized, evidence-based treatment plans.
• Provide gait training, manual therapy, balance work, and therapeutic exercise.
• Implement skin and wound-prevention strategies.
• Educate clients and families to promote long‑term success.
• Document care using our intuitive EMR system.
