Job summary

BAYADA Home Health Care in Colorado is seeking a Part Time, Per Diem Physical Therapist to deliver home health services and help clients regain independence at home. You will perform mobility assessments, create individualized treatment plans, and support patients in a compassionate, patient-centered environment.

Qualifications

• Current Colorado PT license (or eligibility).
• Graduation from an accredited PT program.
• Comfort with EMR systems (training provided).

• Develop individualized, evidence-based treatment plans.
• Provide gait training, manual therapy, balance work, and therapeutic exercise.
• Implement skin and wound-prevention strategies.
• Educate clients and families to promote long‑term success.

What You’ll Do

As a BAYADA PT, you’ll help clients regain independence where they feel safest — at home. You will:

• Implement skin and wound-prevention strategies
• Educate clients and families to promote long‑term success
• Document care using our intuitive EMR system

Compensation & Schedule

• Competitive weekly pay
• Flexible scheduling

• Pay: $65-70 per point, based on qualifications
