BAYADA Home Health Care is currently seeking an experienced Physical Therapist (PT) for a Part-Time or Per Diem opportunity. The role involves performing Home Health Visits to adults and seniors on a per-visit basis in Senior Living Communities throughout Cumberland and Franklin Counties.

The position requires at least one year of prior clinical experience as a licensed Physical Therapist. As a home care Physical Therapist, you will be an essential part of a multidisciplinary healthcare team providing rehabilitative care and skilled nursing, enabling clients to receive necessary medical care while remaining at home.

Responsibilities for a PT - Physical Therapist include:

1. Make home visits to clients within designated geographic areas.
2. Perform diagnostic tests and measurements, including mobility, joint range of motion, transfer status, stability, ambulation patterns, muscle strength and endurance, balance testing, and safety assessments.
3. Implement protective interventions to prevent wounds and promote wound healing using evidence-based dressings and topical agents.
4. Develop and execute individualized care plans, incorporating manual therapeutic exercises, gait training, balance, and other interventions.
5. Continuously assess and update the care plan, collaborating with nursing staff as part of the multidisciplinary team.
6. Educate clients, families, and caregivers on rehabilitative activities and safety to promote health and independence.
7. Document observations, interventions, and evaluations accurately using a modern touchpad tablet.

Qualifications for a PT - Physical Therapist include:

• A valid Pennsylvania Physical Therapist (PT) license.
• At least one year of recent clinical experience as a PT.
• Graduation from an American Physical Therapy Association (APTA)-approved program or equivalent, as evidenced by transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficiency with computers; prior EMR experience preferred.
