BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) for a per diem home health opportunity in Montgomery, Maryland, serving adult and geriatric clients across Alleghany County.

As a home care Physical Therapist, you will be part of a multidisciplinary team providing rehabilitative care and skilled nursing, enabling clients to remain at home.

Responsibilities include:

• Making home visits to clients in designated areas.
• Performing diagnostic tests such as mobility, strength, balance, and safety assessments.
• Developing and implementing individualized care plans, including manual therapy, gait training, and balance interventions.
• Assessing and revising care plans, collaborating with nursing staff.
• Educating clients and families on rehabilitative activities to promote health, safety, and independence.
• Documenting care observations and interventions using electronic tablets.

Qualifications:

• Current Maryland PT license.
• Graduation from an accredited physical therapy program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficiency with computers; experience with EMR preferred.
