Home Health Visits - Physical Therapist

BAYADA is currently seeking an experienced Physical Therapist (PT) for home health visits with adult and senior clients throughout Middlesex and/or Monmouth County.

Applicants must have at least one year of clinical experience as a licensed Physical Therapist. In this role, you will be a key member of a multidisciplinary healthcare team providing rehabilitative care and skilled nursing, enabling clients to remain at home.

Responsibilities for a PT:

• Make home visits to clients in designated areas.
• Perform diagnostic tests and measurements, including mobility, joint range, transfer status, stability, ambulation patterns, muscle strength and endurance, balance, and safety assessments.
• Implement wound care interventions using evidence-based dressings and topical agents.
• Develop and update individualized care plans, including manual therapy, gait training, and balance exercises.
• Assess and revise care plans regularly and collaborate with nursing staff on multidisciplinary care plans.
• Educate clients and their families on rehabilitative care and activities to promote health, safety, and independence.
• Document observations, interventions, and evaluations accurately using a touch pad tablet.

Qualifications:

• Current New Jersey Physical Therapist (PT) license.
• Minimum of one year of recent PT experience.
• Graduation from an American Physical Therapy Association-approved program or equivalent.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficiency in computer skills; EMR experience preferred.
