BAYADA Home Health Care, Inc. is seeking an experienced Physical Therapist (PT) for home health visits in the Upper Montgomery County area.

This full-time/part-time/per-diem role offers a $10,000 sign-on bonus and requires PA PT licensure, with opportunities to develop patient-centered care plans and collaborate with a multidisciplinary team. Responsibilities include in-home assessments, therapeutic exercises, wound care, and documentation via EMR on a state-of-the-art tablet.

Qualifications

• Licensed PA Physical Therapist with current PT license.
• Minimum of one year of recent PT experience.
• Graduation from an APTA/CAPTE-approved program with transcript or diploma.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements for mobility, strength, balance, and safety.
• Implement protective wound care and use evidence-based dressings.
• Develop and revise individualized care plans with therapeutic exercises and gait training.
• Collaborate with nursing in multidisciplinary care plans as appropriate.
• Educate clients and family on rehabilitative care and independence.
• Document observations and evaluations using an EMR on a tablet.

Qualifications for a PT - Physical Therapist include:

• A current PA Physical Therapist - PT license.
• A minimum of one year of recent work experience as a Physical Therapist - PT.
• Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.
