BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) for home health visits in Gaithersburg, Maryland. The role offers either full-time or part-time schedules and several 4-day work week options, with a $10,000 sign-on bonus for full-time employment.

You will be part of a multidisciplinary team delivering rehabilitative care in Montgomery County, with a focus on maintaining patients at home.

• Must hold a current Physical Therapist license (PT).
• Graduation from an American Physical Therapy Association approved program or equivalent.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.

Responsabilidades

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements (mobility, balance, strength, gait, etc.).
• Implement protective interventions to prevent wound formation and promote wound repair.
• Develop and implement individualized care plans including manual therapy and gait training.
• Assess and revise care plans in collaboration with nursing and the multidisciplinary team.
• Educate clients and family members on rehabilitative care and activities for safety and independence.
• Document observations and interventions accurately using a tablet.

BAYADA Home Health Care is currently seeking an experienced Physical Therapist, PT, for a full time or part time opportunity performing home health visits for ourhome health office, located in Gaithersburg, Maryland. This office services adult and geriatric clients on a per visit basis in territories throughout Montgomery County concentrated around Rockville.

As a home care Physical Therapist, you will be an integral member of a multi-disciplinary health care team that provides rehabilitative care and skilled nursing to clients, affording them the opportunity to receive the medical care required to remain at home.

Responsibilities for a PT - Physical Therapist include:

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements, such as the mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments.
• Implement integumentary protective interventions to prevent wound formation and promote wound repair utilizing evidence-based dressings and topical agents.
• Develop and implement appropriate individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
• Continually assess and revise the Physical Therapy - PT care plan, and participate with nursing in the multidisciplinary care plan, as appropriate.
• Educate and instruct clients, family members, or other client representatives, in rehabilitative care and activities necessary to promote the client's health, safety, and independent living.
• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet.

Qualifications for a PT - Physical Therapist include:

• A current MDPhysical Therapist - PT license.
• Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.

Additional Information:

Base Pay, depending upon qualifications and location: $106.000-$125,000 /year

Additional earnings opportunity may be available based on qualifications or other compensation components, if applicable
