BAYADA Home Health Care, Inc. is seeking a Physical Therapist (PT) to provide high-quality home health services across Chester County, PA. The role includes adult and geriatric rehabilitative visits conducted in clients' homes by a multidisciplinary team, enabling patients to stay safely at home.

Formación

• Must hold a Pennsylvania Physical Therapist license.
• Graduate from a program approved by the APTA or AMA committee, as evidenced by transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; EMR experience preferred.

Responsabilidades

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements (mobility, gait, strength, balance, safety).
• Implement wound prevention and repair therapies with evidence-based dressings.
• Develop and implement individualized care plans with therapy interventions.
• Continuously assess and revise the PT care plan with nursing as needed.
• Educate clients, family members, or representatives in rehabilitative care for health and independence.
• Accurately document observations and interventions using an EMR touch tablet.

Responsibilities for a PT - Physical Therapist include:

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements, such as the mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments.
• Implement integumentary protective interventions to prevent wound formation and promote wound repair utilizing evidence-based dressings and topical agents.
• Develop and implement appropriate individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
• Continually assess and revise the Physical Therapy - PT care plan, and participate with nursing in the multidisciplinary care plan, as appropriate.
• Educate and instruct clients, family members, or other client representatives, in rehabilitative care and activities necessary to promote the client's health, safety, and independent living.
• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet.

Qualifications for a PT - Physical Therapist include:

• A current Pennsylvania Physical Therapist - PT license.
• Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.

Base Pay, depending upon qualifications, status and subject to negotiation:
$65.00 - $75.00 per point ($101,400 - $117,000 annual earning potential)
