About BAYADA Home Health Care

We are seeking an experienced Physical Therapist (PT) for a Full Time or Part Time position providing Home Health visits throughout Mahoning County. Our office serves adult and geriatric clients on a per-visit basis.

Role Overview

As a home care Physical Therapist, you will be an essential part of a multidisciplinary healthcare team that offers rehabilitative care and skilled nursing, enabling clients to remain at home.

Responsibilities

1. Make home visits to clients in designated areas.
2. Perform diagnostic tests such as mobility, joint range of motion, transfer status, stability, ambulation patterns, muscle strength and endurance, balance, and safety assessments.
3. Implement protective interventions to prevent wound formation and promote healing, using evidence-based dressings and topical agents.
4. Develop and update individualized care plans, including manual exercises, gait training, and balance interventions.
5. Assess and revise care plans regularly and collaborate with nursing staff as part of the multidisciplinary team.
6. Educate clients and their families on rehabilitative activities to promote health, safety, and independence.
7. Document all observations and interventions accurately using a touchpad tablet.

Qualifications

• Current Ohio Physical Therapist (PT) license.
• Graduation from an accredited program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficiency with computers; experience with electronic medical records (EMR) preferred.
