Job summary

VitalCaring — a home health provider — is seeking a Physical Therapist (PT) for PRN, in Gautier, Mississippi. You will deliver patient-centered home-based therapy, evaluate functional status, and collaborate with physicians and the care team to restore mobility and independence.

You will educate patients and families, document progress in the EMR, and participate in case conferences. Prior home health preferred; CPR and Mississippi PT license required.

Qualifications

• Graduate of an accredited Physical Therapy program (DPT or Master’s).
• Active PT license in the state of practice (Mississippi).
• 1–2 years of home health experience preferred.
• CPR certification and reliable transportation required.

Responsibilities

• Evaluate patients’ functional status, mobility, and rehabilitation potential.
• Develop and implement individualized care plans with the care team.
• Provide hands-on therapy to improve mobility, strength, balance and independence.
• Educate patients and caregivers on treatment plans, exercises, and safety.
• Monitor progress, adjust plans, and participate in discharge planning.
• Document care and progress accurately in the EMR; attend team meetings.
• Supervise PTAs in accordance with state guidelines; support caregivers.

What You’ll Do

Deliver Patient-Centered Care

• Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential
• Develop and implement individualized care plans in collaboration with physicians and the care team
• Provide hands-on therapy to improve mobility, strength, balance, and independence
• Educate patients and caregivers on treatment plans, exercises, and safety strategies

Drive Outcomes & Recovery

• Monitor patient progress and adjust treatment plans as needed
• Support safe transitions of care and contribute to discharge planning
• Identify additional patient needs and collaborate with interdisciplinary teams
• Partner closely with nurses, therapists, and physicians to ensure coordinated care
• Document patient care and progress accurately and timely within the EMR
• Participate in case conferences and team meetings

Lead & Support

• Supervise Physical Therapist Assistants (PTAs) in accordance with state guidelines
• Provide guidance and education to caregivers and support staff

What You Bring

• Graduate of an accredited Physical Therapy program (DPT or Master’s)
• Current PT license in the state of practice
• Minimum 1–2 years of experience (home health experience preferred)
• Strong clinical assessment and critical thinking skills
• Excellent communication and patient education abilities
• Current CPR certification
• Valid driver’s license, reliable transportation, and auto insurance
