Job Description

As a Physical Therapist, you will develop and implement care plans, evaluate patient needs, and collaborate with team members to ensure optimal outcomes. Your role involves direct patient care, documentation, leadership, and community engagement. The environment can be physically demanding and may require irregular hours, including call shifts.

Responsibilities include:

• Promoting Elara Caring’s mission and policies
• Providing direct patient care and adjusting care plans
• Designing and implementing patient care plans
• Documenting evaluations and treatment plans
• Leading and supporting staff
• Engaging with the community to promote physical therapy
• Handling physical demands of the role, including lifting and transferring patients
• Potentially working irregular hours, including call shifts

Minimum Requirements:

• Graduate of an APTA-approved physical therapy program
• Current, unrestricted PT license
• At least one year of healthcare experience
• Willingness and ability to travel within the coverage area
• Dependable vehicle, valid driver’s license, auto insurance
• Ability to travel up to 50%
• Physical ability to perform job duties, including lifting 50-100 lbs

Reporting to the Branch Director or designated supervisor, you will be part of a team dedicated to exceptional patient care.
