Job summary

Assisted Home Health & Hospice seeks a licensed Physical Therapist (PT) for our Thousand Oaks home health office. Full-time preferred, with per diem options available. You will assess patients at home, perform OASIS when appropriate, and guide therapeutic interventions and equipment needs.

The role emphasizes thorough functional assessments, gait analysis, ROM testing, and patient-centered therapy to support independence in daily living. California licensure is required.

Qualifications

• Licensed Physical Therapist in the State of California.
• Experience with home health assessments and OASIS knowledge is a plus.

Responsibilities

• Complete initial and ongoing assessments to determine level of functioning, including OASIS assessments at appropriate time points.
• Execute assessments using procedures such as manual and electrical muscle examinations, functional evaluations, gait analysis, sensory testing, ROM measurements, and movement analysis.
• Provide therapeutic treatments and evaluate equipment needs as applicable.

Visit rates (all below rates including $11 for mileage reimbursement):

$146 per/SOC
$101 per/Eval
$91 per/Routine
