Job summary

Assisted Home Health & Hospice is seeking a per diem Occupational Therapist to support our Thousand Oaks, CA home health team. You will evaluate patients, develop and implement individualized treatment plans, and collaborate with nurses and other therapists to maximize independence and function.

Requirements include graduation from an accredited OT program, active OT license, and at least one year of experience.

Qualifications

• Graduate from an accredited OT program and be licensed to practice.
• Minimum of one year of occupational therapy experience.
• Experience in home health or multidisciplinary settings is a plus.
• Ability to collaborate with nurses and therapists to maximize patient independence.

Responsibilities

• Team with nurses, physical therapists and other medical professionals to provide therapeutic care.
• Evaluate function and initiate an OT treatment plan to relieve pain and restore independence.

The impact you will make:

• Team with nurses, physical therapists and other medical professionals in order to provide our patients with the best possible therapeutic care, with the goal of increasing their independence and their overall success in maximizing their everyday living and working skills
• Evaluate the level of function and initiate a plan of treatment for Occupational Therapy to assist with relieving pain, preventing disability, developing body skills, restoring function and maintaining maximum capabilities through the use of graded functional Occupational Therapy activities

Who you are:

• Graduate of an accredited occupational therapy curriculum
• Licensed as an Occupational Therapist
• At least one year of experience in occupational therapy
• Experience in a variety of settings a plus
