Responsibilities of a Home Health Physical Therapist

• Provide home-based physical therapy to patients with mobility, strength, and functionality challenges
• Collaborate with a home health team including RN, LPN, CNA, HHA, OT, and SLP
• Assess, document, and modify care plans as needed
• Assist patients with exercises and hands-on therapy to manage pain and improve mobility
• Educate patients and families about care plans, exercises, and self-care

Must-Haves for Home Health Physical Therapists

• Graduate of an accredited Physical Therapist Program and active PT license in IL
• Minimum of 1 year of physical therapy experience, preferably in home healthcare
• CPR certification
• Knowledge of home health regulations
• Strong clinical judgment, interpersonal skills, dexterity, and compassion
