Job Summary:

The PT administers physical therapy in patients' homes, following physician orders and care plans, under supervision of the Director of Clinical Services/Nursing Supervisor/Rehab Director.

Responsibilities include:

• Providing physician-prescribed physical therapy
• Assessing and improving residual disabilities
• Participating in care planning and documentation
• Educating patients and families on home programs
• Completing assessments and reports in compliance with agency procedures

Qualifications:

• Graduate of an accredited physical therapy program
• Current licensure in Missouri and Illinois
• Preference for 2+ years of experience
• Ability to work independently and exercise judgment

Physical Requirements:

• Ability to perform physical activities including lifting and bending
• Ability to work long periods standing and involved in physical activity
