Location: This position will conduct patient home care visits in the Polk County area.

The role you'll contribute:

The Home Health Physical Therapist (PT) Care Manager is a professional who coordinates and directs home care services based on individual patient needs. The PT Care Manager manages a caseload of home health patients requiring therapy, utilizing advanced assessment, teaching, and decision-making skills. Responsibilities include ensuring appropriate referrals, interdisciplinary conferencing, and proper documentation. The PT evaluates patients for home health appropriateness, develops care plans with physicians, educates patients and families, and monitors outcomes to achieve optimal patient and family goals. The PT also participates in performance improvement initiatives and standard of care programs.

• Coordinates and manages care for a caseload of home patients requiring physical therapy, providing comprehensive assessment, planning, implementation, and evaluation.
• Prioritizes caseload based on patient and family needs, optimizing daily schedules for productivity and best practice visit utilization.
• Performs functional assessments measuring activities of daily living, documenting results accurately.
• Develops patient-specific, feasible care plans with measurable goals, considering physical, cultural, psychosocial, spiritual, age, and educational needs.
• Reassesses patients periodically, providing therapy, evaluating effectiveness, and documenting progress.
• Records observations and reports patient responses and condition changes to physicians.

Qualifications:

• Master's Degree in Physical Therapy
• 1+ year relevant clinical physical therapy experience
• Valid Physical Therapy license in the state of practice
• Valid driver’s license with current car insurance
• Active American Heart Association BLS certification

Preferred qualifications:

• Recent experience in a Medicare-certified home health agency as a case manager
• OASIS Specialist – Clinical certification
