Location: This position will conduct patient home care visits in the Hillsborough County area.

The role you’ll contribute :

The Home Health Physical Therapist (PT) Care Manager is a professional therapist who coordinates and directs the home care patient’s services based on individual patient needs. The PT Care Manager manages a caseload of home health patients requiring therapy as the primary service, evaluating patients for appropriateness of home health, developing care plans with physicians, educating patients and families, and monitoring outcomes. The PT also identifies performance improvement opportunities and assists in implementing quality initiatives.

• Coordinates and manages care for a caseload of home patients requiring physical therapy as the primary skill. Provides assessment, planning, implementation, and evaluation for this caseload.
• Sets priorities and adapts to changing patient needs, optimizing visit schedules for productivity and best practice utilization.
• Performs functional evaluations assessing activities of daily living, documenting results in the clinical record.
• Formulates patient-specific care plans with measurable, realistic goals considering physical, cultural, psychosocial, spiritual, age-specific, and educational needs.
• Reassesses patients periodically, providing therapy, evaluating progress, and documenting outcomes.
• Records activities and findings, reports patient reactions and condition changes to physicians, and adjusts care plans as needed.

Qualifications

The expertise and experiences you’ll need to succeed :

• Master's Degree in Physical Therapy
• 1+ year relevant clinical physical therapy experience
• Physical Therapy licensure in the state of practice
• Valid driver’s license with current car insurance
• Active American Heart Association BLS certification

Preferred qualifications:

• Recent experience in a Medicare-certified home health agency as a case manager
• OASIS Specialist – Clinical certification
