Whether recovering from an acute stay away from home or a short term illness, helping patients learn to navigate the safety challenges that their own home presents is both valuable and rewarding.
As a Banner Home Health Physical Therapist, you will enjoy working in sub-assigned geographic territories with smaller travel distances than many other agencies. You will enjoy an autonomous but supportive work environment and have the option to work pay per visit or hourly. We offer mileage reimbursement and will provide you a company cell phone and a tablet for documentation in our electronic medical record system (Home Care Home Base).
This position is responsible for the evaluation, planning, administration, and supervision of Physical Therapy treatment in accordance with physician referral, state and national organizational standards, regulatory agencies, and established departmental policies and procedures.
Must have graduated from an APTA accredited school of Physical Therapy, or equivalent. Must possess a current Physical Therapy license within the State of practice. BLS certification required.
Additional related education and/or experience preferred.
