Whether recovering from an acute stay away from home or a short term illness, helping patients learn to navigate the safety challenges that their own home presents is both valuable and rewarding.

As a Banner Home Health Physical Therapist, you will enjoy working in sub-assigned geographic territories with smaller travel distances than many other agencies. You will enjoy an autonomous but supportive work environment and have the option to work pay per visit or hourly. We offer mileage reimbursement and will provide you a company cell phone and a tablet for documentation in our electronic medical record system (Home Care Home Base).

Banner Home Health offers an opportunity to treat patients with a wide variety of clinical conditions and goals– post-op orthopedic patients, neuro rehab, cardiac issues, and more. This position will provide visits in the East Valley with home visits primarily on the weekends (Saturdays and Sundays), with flexible schedule options for additional workdays.

Position Summary

This position is responsible for the evaluation, planning, administration, and supervision of Physical Therapy treatment in accordance with physician referral, state and national organizational standards, regulatory agencies, and established departmental policies and procedures.

Core Functions
  1. Evaluates patient's functional abilities and limitations. Establishes a plan of care and treatment goals.
  2. Implements individualized treatment plan considering the patient's needs. Involves patient and family in the treatment process.
  3. Documents according to professional practice guidelines and department policies.
  4. Communicates pertinent patient information to ensure optimal patient outcomes.
  5. Exhibits adaptability in assignments and takes initiative to assist in other patient care areas.
  6. Demonstrates appropriate supervision and delegation of personnel.
  7. Participates in departmental/facility process improvement.
  8. Interacts with all levels of staff in various departments concerning patient care.
Minimum Qualifications
  1. Graduated from an APTA accredited school of Physical Therapy, or equivalent.
  2. Possess a current Physical Therapy license within the State of practice. BLS certification required.
  3. Must have the ability to become proficient with computer software applications commonly used in an automated patient care environment.
Preferred Qualifications

Additional related education and/or experience preferred.
