Argan Home Care is seeking a passionate Physical Therapist to join our home-based care team serving Punta Gorda, Port Charlotte, and Englewood, FL. Deliver personalized therapy in patients’ homes with physician-guided plans, while promoting independence and mobility.

We value independent, patient-centered clinicians with excellent communication and organizational skills who can travel throughout the Port Charlotte area.

Formación

• Valid Florida Physical Therapist license in good standing.
• Graduate of an accredited Physical Therapy program (Master's or Doctorate preferred).
• Home health or outpatient experience preferred; new graduates encouraged to apply.
• Strong knowledge of physical rehabilitation, therapeutic exercise, and manual therapy techniques.
• CPR/BLS certification required or ability to obtain upon hire.
• Reliable transportation and valid driver's license for home visit travel throughout Port Charlotte, FL.

Responsabilidades

• Evaluate patients' physical conditions and develop individualized treatment plans to restore function and improve mobility.
• Provide skilled physical therapy services in patients' homes in accordance with physician orders and plan of care.
• Monitor and document patient progress, adjusting treatment plans as needed to ensure optimal outcomes.
• Educate patients and family members on therapeutic exercises, safety techniques, and home programs.
• Collaborate with interdisciplinary care teams including physicians, nurses, and other healthcare professionals.
• Maintain accurate and timely clinical documentation in compliance with regulatory and agency standards.
• Participate in discharge planning and coordinate continuity of care for patients transitioning between levels of care.
