The role emphasizes independent work in a home setting, strong clinical assessment, and proficient use of Homecare Homebase for documentation and coordination of care.

Qualifications

• Current Pennsylvania Physical Therapist license.
• Home health experience preferred.
• Experience with Homecare Homebase preferred.
• Strong clinical assessment and communication skills.
• Ability to work independently while collaborating with the interdisciplinary care team.
• Reliable transportation and ability to travel throughout the assigned territory.

Responsibilities

• Provide skilled physical therapy services to patients in their homes.
• Complete evaluations and develop individualized plans of care.
• Provide therapeutic interventions focused on mobility, strength, balance, safety, and functional independence.
• Educate patients, caregivers, and families.
• Coordinate care with physicians, nursing, therapy, and other members of the home health team.
• Complete timely and accurate documentation using Homecare Homebase.
