Job summary

Blue Line Search Group is seeking a Home Health Physical Therapist in Lorain County, Ohio. You will evaluate patients, develop personalized plans, and provide PT services in patients' homes.

Ideal candidates hold an active PT license, have direct patient care experience, and can manage a flexible, independent schedule. EHR experience and strong communication are valued assets.

What You'll Do

• Evaluate patients and develop individualized treatment plans in their homes.
• Provide skilled physical therapy interventions and monitor patient progress.
• Communicate with patients, families, and physicians about care plans.
• Manage your own daily schedule and caseload with minimal travel time.
• Complete accurate clinical documentation in a timely manner.
• Build strong, meaningful relationships with patients and care team members.
• Maintain high clinical and professional standards in all patient interactions.

What You'll Need

Instead of hourly pay, each visit earns points based on time, complexity, and clinical responsibility. You control your earning potential—take more visits for higher income, or maintain a balanced caseload. Specific point values are discussed during interviews.
