Elara Caring is seeking a Physical Therapist to serve patients throughout the Danbury, Connecticut territory in a home health setting. You will deliver one-on-one care and independently manage patient visits with support from an experienced clinical team.

The role emphasizes mobility restoration, functional independence, and education for patients and caregivers, with travel across the Danbury territory and a strong focus on safe home-based therapy.

Qualifications

• Graduate of an American Physical Therapy Association-approved physical therapy program.
• Active and unrestricted Connecticut Physical Therapist license.
• At least one year of healthcare experience.
• Ability to travel 50% in the Danbury territory.
• Strong communication and patient-care skills.

Responsibilities

• Evaluate patients’ mobility, strength, safety, and functional needs.
• Develop and implement individualized treatment plans.
• Help patients improve mobility and safely perform daily activities.
• Educate patients and caregivers on exercises, equipment, and safety techniques.
• Monitor patient progress and adjust treatment plans as needed.
• Communicate changes in condition to physicians and care team members.
• Complete accurate and timely clinical documentation.
• Collaborate with nurses, physicians, therapy professionals, and the care team.
• Support patients in reaching their highest possible level of independence.

What You’ll Bring

• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment

Home health experience is valuable, but candidates with transferable clinical experience are encouraged to apply.
