Elara Caring is seeking a compassionate Physical Therapist to deliver home health services across Stamford and surrounding communities. You will work independently in patients’ homes while collaborating with a clinical team to improve mobility, safety, and quality of life.

Qualifications

• Graduate of an American Physical Therapy Association‑approved physical therapy program.
• Current and unrestricted Connecticut Physical Therapist license.
• At least one year of experience in a healthcare setting.
• Willingness to travel 50% and cover Stamford area.
• Ability to frequently lift, push, pull, and transfer patients up to 50 pounds.

Responsibilities

• Evaluate patients’ mobility, strength, balance, functional abilities, and home safety needs.
• Develop and implement individualized plans of care.
• Provide hands-on therapy to improve mobility and independence.
• Establish measurable treatment goals and monitor progress.
• Adjust treatment plans based on changes in condition and outcomes.
• Educate patients, caregivers, and family on exercises, equipment, and safety.
• Coordinate care with physicians, nurses, therapists, and other care team members.
• Direct and support therapy assistants when applicable.
• Complete evaluations, treatment notes, and clinical documentation.
• Serve as a clinical resource and advocate for patients receiving therapy at home.

What You’ll Bring

• Graduate of an American Physical Therapy Association‑approved physical therapy program
• Current and unrestricted Connecticut Physical Therapist license
• At least one year of experience in a healthcare setting
• Strong assessment, communication, organization, and documentation skills
• Ability to work independently in patients’ homes
• Willingness to travel throughout Stamford and the assigned coverage area
• Must be able and willing to travel 50%
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver’s license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations
