Elara Caring is seeking a Physical Therapist Assistant to join our home health team in the Hamden, CT area. You will deliver PTA services in patients' homes under the supervision of a Physical Therapist, following established Plans of Care and documentation standards.

Qualifications

• Graduate of an accredited PTA program approved by the state licensing board.
• Current, unrestricted PTA license valid for the state of work.
• Willingness to travel within branch/office coverage area and travel 50% of the time.

Responsibilities

• Provide direct PTA services per Plan of Care and physician orders to ensure quality and continuity of care.
• Follow Plan of Care established by Physical Therapist and report changes to supervising PT.
• Instruct patients and families in home exercise programs and safe use of devices.
