Zusammenfassung

Amedisys is seeking a Physical Therapist to provide hands-on in-home care for geriatric patients on a weekend Baylor Day Shift with paid training and mileage reimbursement. You’ll work 1:1 in patients' homes, delivering interventions, reexaminations at least every 30 days, and discharge planning in a flexible schedule.

Required: PT license in the assigned state, CAPTE-accredited degree, CPR, and at least 1 year of home health experience (mentoring available if less).

Qualifikationen

• Current license to practice physical therapy in the assigned state.
• Graduate from a CAPTE-accredited physical therapy program.
• One year of home health PT experience preferred.
• If less than a year, approval from regional leaders and mentoring may be required.
• Current CPR certification.

Responsibilities

• Responsible for helping patients maintain, restore and improve movement, activity and function.

• Provides interventions, conducts reexaminations (every 30 days, at minimum), modifies interventions as necessary, and performs discharge planning.
• Other duties as assigned.

Qualifications

• Current license to practice physical therapy specific to the state you're assigned to work in.

• Graduate from an accredited physical therapy educational program as set by the Commission on Accreditation of Physical Therapy Education (CAPTE).
• One year related physical therapy experience for PTs entering home health care to ensure that knowledge and skills are sufficient to safely provide physical therapy services to patients.
• If less than one year of experience, approval from regional clinical leaders is required as well as appropriate mentoring and/or participating in a residency program.
• Current CPR certification.
