Job summary

Amedisys is hiring a Physical Therapist to provide hands-on care in patients' homes across Georgia. This weekend day shift role focuses on helping geriatric patients improve movement and function with consistent reexaminations and discharge planning.

Mileage reimbursement supports a flexible, home-based schedule and a competitive pay range. Responsibilities include initiating interventions, adjusting plans as needed, and coordinating care with families.

Qualifications

• Must hold a valid PT license for the state (GA).
• Graduate from CAPTE‑accredited PT program.
• At least 1 year of PT experience in home health preferred.
• If less than 1 year, mentoring and residency may be required.

Responsibilities

• Help patients maintain, restore and improve movement and function.
• Conduct interventions, reexaminations every 30 days, modify plans as needed, perform discharge planning.
• Provide in-home PT services and care coordination.

Responsibilities

• Responsible for helping patients maintain, restore and improve movement, activity and function.


• Provides interventions, conducts reexaminations (every 30 days, at minimum), modifies interventions as necessary, and performs discharge planning.


• Other duties as assigned.

Qualifications

• Current license to practice physical therapy specific to the state you're assigned to work in.


• Graduate from an accredited physical therapy educational program as set by the Commission on Accreditation of Physical Therapy Education (CAPTE).


• One year related physical therapy experience for PTs entering home health care to ensure that knowledge and skills are sufficient to safely provide physical therapy services to patients.


• If less than one year of experience, approval from regional clinical leaders is required as well as appropriate mentoring and/or participating in a residency program.


• Current CPR certification.
