Job summary

TheraCare seeks a licensed Physical Therapist (PT) for a PRN home health role across South Dallas, Lancaster, DeSoto, Duncanville, TX. Provide one-on-one in-home therapy, perform OASIS assessments, and develop individualized plans of care to improve mobility and independence.

You’ll educate patients and families, document in the EMR, and collaborate with an interdisciplinary team while enjoying flexible scheduling and autonomy in the home setting.

Qualifications

• DPT from an APTA-accredited program.
• Active PT license in Texas or eligible for Compact Licensure.
• Home health experience (1 year preferred).
• Current CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Perform home health PT evaluations and re-evaluations, including OASIS assessments.
• Develop, implement, and update Plans of Care based on goals and orders.
• Deliver skilled PT treatment for orthopedic, neurological, cardiac, and post-surgical conditions.
• Educate patients and caregivers on home exercise programs and safety.
• Monitor progress and modify treatment plans as needed.
• Document accurately in the EMR at the point of care.
• Supervise PTAs and home health aides per state regulations and policy.
• Coordinate with physicians, nurses, and case managers.
