Creekside Post-Acute is seeking a dedicated Physical Therapist (PT) to evaluate residents and develop individualized treatment plans in a skilled nursing setting in San Jose, CA. The role requires a DPT, California PT license, and experience in post-acute or geriatric rehabilitation.

Qualifications

  • Earn a Doctor of Physical Therapy (DPT) degree from an accredited physical therapy program.
  • Pass both national and state board examinations required for licensure.
  • Hold a current and unrestricted California Physical Therapist license.
  • Previous experience in Skilled Nursing, post-acute care, rehabilitation, or geriatric settings preferred.

Responsibilities

  • Conduct comprehensive physical therapy evaluations based on physician orders, resident needs, and clinical findings.
  • Develop individualized treatment plans with measurable goals based on each resident's functional needs.
  • Provide skilled physical therapy interventions to improve strength, mobility, balance, coordination, endurance, and functional independence.
  • Provide therapeutic exercises, gait training, transfer training, mobility activities, and other appropriate interventions.
  • Monitor resident progress and modify treatment plans as clinically appropriate.
  • Educate residents, families, and caregivers regarding exercises, mobility, safety, and strategies to support continued progress.
  • Maintain accurate, timely, and complete clinical documentation.
  • Participate in care plan meetings and collaborate with the interdisciplinary team.
  • Communicate effectively with nursing, physicians, occupational therapists, speech therapists, rehabilitation staff, residents, families, and caregivers.
  • Promote resident safety, dignity, independence, and quality of life.
  • Follow facility policies, professional standards, and applicable federal and state regulations.
  • Maintain a safe and therapeutic treatment environment.
  • Perform other duties as assigned.

Position Summary

Creekside Post-Acute is seeking a skilled and compassionate Physical Therapist (PT) to join our rehabilitation team. The Physical Therapist will evaluate residents, develop individualized treatment plans, provide skilled therapy services, and monitor progress to help residents achieve their highest level of function, mobility, independence, and quality of life.

The ideal candidate is passionate about patient-centered care and enjoys working collaboratively with residents, families, caregivers, and an interdisciplinary healthcare team.

Essential Duties and Responsibilities

Conduct comprehensive physical therapy evaluations based on physician orders, resident needs, and clinical findings.

Provide therapeutic exercises, gait training, transfer training, mobility activities, and other appropriate interventions.

Assess and recommend appropriate assistive devices, adaptive equipment, and mobility strategies.

Educate residents, families, and caregivers regarding exercises, mobility, safety, and strategies to support continued progress.

Maintain accurate, timely, and complete clinical documentation.

Participate in care plan meetings and collaborate with the interdisciplinary team.

Communicate effectively with nursing, physicians, occupational therapists, speech therapists, rehabilitation staff, residents, families, and caregivers.

Promote resident safety, dignity, independence, and quality of life.

Follow facility policies, professional standards, and applicable federal and state regulations.

Maintain a safe and therapeutic treatment environment.

Perform other duties as assigned.

Education & Qualifications
