Job Overview

Absolute Skilled Home Care provides skilled nursing, therapy services, social services, and home health aides in the home under the direction of the patient's Primary Care Physician.

Responsibilities

• Provide a wide range of physical therapy services to patients in the home care setting.
• Focus on patient assessment, quality care, monitoring patient status and compliance, teaching patients and caregivers, and discharge planning.
• Deliver care in accordance with Ohio Standards of Practice, company policies, and regulatory requirements.

Qualifications

• Graduation from an accredited Physical Therapy program.
• Valid license in the State of Ohio.
• Previous experience in a home care setting is preferred.
• Minimum of one year experience in an acute care or rehabilitation setting.
• Ability to transfer, maneuver, and lift incapacitated clients in the home setting.
